Unlike other respondants, I'm not convinced that it is a PHP problem
at all. AddType is defined in mod_mime
(http://httpd.apache.org/docs/2.0/mod/mod_mime.html#addtype). Could it
be possible that it is not loaded? Check with /server-info (if it is
enabled) or see (if Ubuntu does like Debian) /etc/apache2/mods-enabled
if there is a "mime.load" or similar.
