First, ensure you have reachability to the TFTP server:

ping 172.16.0.4

Then make sure the TFTP server is running.

What TFTP server are using? Are you running it on UNIX/Linux 
or Windows?

In either case, ensure the TFTP server is configured to have 
access to your TFTP directory, and that the TFTP directory 
(presumably where you stored the IOS image) actually does 
exit and that the IOS file is readable by all.
