I suggest you start by isolating the call manager in your
troubleshooting. Connect an IP phone back to back to the router and
originate a call to B,C and D. If the call fails, please give details on
the codec in use, signalling etc etc actually just send the relevant
bits of your configuration for all the concerned devices including the
PBX. While you are at it, let us know the call manager release and what
interface you have on the PBX to the router.
 
If you have a problem conecting the phone directly to the router due to
some logistics beyond you, list all the route and gateway patterns on
the call manager, or tell us what entries you have under route pattern
configuration and send them alongside the other configuration.
