Well, 2 things that really stand out:
- Upgrade your IOS version; what you have is very
old, unless your supervisor module is ancient too.
- Don't use VTP, it causes more problems than it
solves. Your scenario is typical, where a
different switch (supervisor module, in your case)
is introduced into the network.
